What is the Community Health Hub?
The Community Health Hub (CHH) is an online resource for discovering health metrics and community priorities in the New Hampshire and Vermont region. The Health Hub serves to communicate to the community in lay language about the value of community-engaged research. In addition, it helps researchers to tell the story of their research and invite the community to report on their experience. The site will evolve and expand to serve the changing needs of our communities and researchers. We welcome your feedback, questions or suggestions at any time by contacting us.

Health Dashboards

Dartmouth-Hitchcock works with many community organizations to improve the health and well-being of the communities we serve, addressing priorities identified in regional Community Health Needs Assessments (CHNAs) and other regionally identified needs. Every three years Dartmouth-Hitchcock identifies what we intend to do to address these needs in our Community Health Improvement Plan (CHIP). The dashboards below include health, public health, and program measures Dartmouth-Hitchcock and other organizations such as the Public Health Council of the Upper Valley and the Greater Sullivan County Public Health Region use to evaluate our work to implement this Community Health Improvement Plan.
Contact us with questions about the Dartmouth-Hitchcock CHIP dashboards or if your organization would be interested in participating in this regional community health dashboard initiative.
